Position Description

The Regional Manager
– Quality is responsible for the planned, ongoing, and systematic gathering and assimilation of data relating to the delivery of quality patient care that is measured and validated.

The RQM provides regional support and education to health care professionals related to clinical practice standards, QAPI, HQRP, CHAP, CMS, and State regulations.

The RQM assists with regional monitoring for compliance with applicable regulations and conditions of participation for the hospice patient plan of care.

Essential Duties and Responsibilities

  • Assists with review of medical records and clinical notes to evaluate compliance
  • Assists with maintaining an agency wide program for the surveillance, identification, prevention, control and investigation of infectious and communicable diseases
  • Assists with Clinical Record Review data collection, reports,
  • Conducts internal mock surveys, attends IDG meetings, led State, federal and CHAP survey responses as requested
  • Reports findings to Administrator and/or Supervising Nurse
  • Provides follow up with staff and provides education and training regarding findings related to clinical record review
  • Responsible for the hospice quality assessment and performance improvement (QAPI) for assigned regions:
  • Assists with performance improvement activities
  • Initiate chart reviews to identify information supporting the need for continued hospice care services
  • Holds conferences with members of the hospice care team including Nurses, Physicians, and other team members as indicated, to identify status, clarification of goal attainment or revision, changes in services of intensity, treatment, medications, etc.
  • Documents all conferences with interdisciplinary team members in the medical record as indicated
  • Completes required reports related to quality (i.e.

    Quality Assurance and Process Improvement initiatives and monthly quality measures, etc.) in the required time frame

  • Assures compliance with Federal and State guidelines
  • Inform the Director
    – Quality of noticeable trends or relevant issues
  • Ability to travel within assigned region
  • Attends all mandatory in-services and meetings provided by agency
  • Participates in Quality Assessment and Performance Improvement (QAPI) program through submission of data focusing on indicators related to improved palliative outcomes and serving on QAPI team
  • Participates in Performance Improvement activities
  • Understands and apply basic infection control principles and procedures.

    Will contact supervisor with any concerns related to infection control

  • Follows agency policies and procedures to operate and furnish services in compliance with all applicable Federal, State, and local laws and regulations
  • Inform the Director
    – Quality of noticeable trends or relevant issues
  • Ability to travel within assigned region
  • Performs additional duties as assigned

REQUIRED Knowledge, Skills, and Experience

  • Bachelor’s Degree in Nursing
  • Active RN License
  • Possession or ability to obtain Certified Hospice and Palliative Nurse Certification within 1 year of hire
  • Must maintain a valid driver’s license and good driving record
  • Three years of administrative or supervisory experience in home health/hospice care or related health program (hospital, nursing facility, etc.)
  • Two years’ experience as a QA nurse of a Medicare-certified hospice agency
  • A working knowledge of Community Health Accreditation Program (CHAP) on Accreditation, Quality Assurance, and Hospice knowledge required
  • Experience with State Surveys
  • Possesses a working knowledge of basic statistics including trending, problem identification, action plans and evaluation
  • Excellent problem solving skills
  • Excellent written and verbal communication with personnel of all levels.
  • Ability to travel up to 80% of the time
  • Ability to set objectives and develop action plans
  • Proficiency in personal computer use, including e-mail, clinical, word processing, spreadsheet and presentation software

PREFERRED Knowledge, Skills, and Experience

  • Possess Certified Hospice and Palliative Nurse (CHPN) certification
  • Lean Six Sigma Yellow Belt
    – Understand basic Lean Six Sigma concepts
