Intercommunity Action

This is a Full-time position in Philadelphia, PA posted October 22, 2021.

PURPOSE The Geriatric Clinician is responsible for providing clinical assessment and family therapy to clients in the designated program area.

Services are offered at client homes, and/or Senior Community Centers according to client need and may be provided in collaboration with other psychiatric, medical, or social service providers the client is or becomes involved with.

FUNCTIONSPerforms comprehensive psychosocial assessment including a mental status exam and diagnosis on all clients referred to his/her caseload.

Based on the assessment, identifies a problem list and develops and implements treatment plan.

Provides individual and/or family therapy to assessed clients using a broad range of therapeutic approaches including, but not limited to, psychoeducation, trauma informed, cognitive, and behavioral therapies to address target symptoms and problems.

Assesses client need for additional psychiatric, medical, and/or social services and refers to Care Coordinator.

With client consent, works in collaboration with all service providers involved with client or to whom client has been referred.

Maintains effective collaborative working relationships with referral sources.

Travels to and between senior centers and client homes for service delivery at those sites.

Maintains clinical records within specified time frames utilizing Agency’s electronic medical records in Accordance with Agency policies and procedure, state and County regulations, and managed health care requirements.

Completes necessary trainings to maintain credentials and license(if applicable) and to meet requirements set forth by State and county oversight agencies and payors including Community Behavioral Health.

Adheres to safe work practices.

Must function in full compliance with all applicable policies and procedures, federal, state and local laws and Interact’s Code of Conduct.

Must participate in Interact’s Compliance Program including reporting suspected violations of the compliance program or applicable laws or regulations to supervisor/manager/director or the Compliance Officer.

All other duties as assigned.

QUALIFICATIONS Master’s degree in social work or related human service field plus one (1) year of experience in behavioral health services.

Certified in one or more evidence based treatments and/or willingness to participate in Interact’s evidence based treatment training program.

Personal automobile available for daily work use.

Possession of a valid driver’s license from state of residence required.

A minimum of one (1) year driving experience with a driving record free of at-fault accidents and/or moving violation within the past year preferred.

Excellent oral, written, and interpersonal communication skills.

Demonstrates basic PC skills including keyboarding, working knowledge of WORD and comfort with web based environments, experience with electronic medical records preferred.

Ability to within an interdisciplinary team.

Excellent organizational skills and ability to work independently.

Satisfactory criminal, child abuse, and FBI clearances in accordance with PA Acts 13, 33, and 73.
