Delivers professional nursing services to patient and families assigned, and demonstrates the ability to assess, to plan, to evaluate and to implement nursing care with attention to the special needs of the agency.

Works in accordance with the Teams Plan of Care and written agency programs and policies.

The intent of this job description is to provide a summary of the major duties and responsibilities of this position and shall not be considered as a detailed description of all the work requirements that may be inherent in the position.

nn JOB DUTIES AND RESPONSIBILITIES nn Evaluates and does an assessment on all assigned patients in the inpatient facility.

Considers psychosocial, environmental, spiritual and financial needs.

Communicates with the patients physician and/or hospice Medical Director, both orally and in writing as required by the patients condition and agency policy.

Cooperates with the Volunteer Coordinator and is available to hospice volunteers for counsel, supervision, advice and training when dealing with the patients and families.

Assumes general personal responsibility to keep informed of current changes and trends affecting Hospice care and professional nursing functions.

Determines eligibility for admission; obtains financial and statistical data; formulates initial care plan according to the patients condition.

Carries primary responsibility for providing comprehensive nursing care, health teaching, guidance and supervision to the individual and family while in inpatient facility.

Supervises patient care given by Hospice Aides.

Maintains and uses patient clinical records, reports and other written communications according to agency policy and directives.

Clinical records are written daily.

Records make clear goals and plans with emphasis on the family oriented approach to patient care.

Records reflect contact with physicians and other agencies needed.

Participates in the development and operation of the agency by: Keeping administration informed on changing community needs and lack of community resources.

Participation in the orientation and guidance of new staff.

Interpreting agency service to families and community groups.

Contributing to evaluation of service programs.

Evaluating effectiveness of his/her own service and contributing to the evaluation of and continuing development of staff members.

Participating in the development of inservice programs and group teaching for staff and other groups, as assigned Acts as triage nurse in accordance with agency on-call policy.

Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.

Demonstrates/models the Networks Service Excellence Standards of Performance in interactions with all customers (internal and external).

Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.

Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practice.

Complies with Network and departmental policies regarding attendance and dress code.

Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

nn PHYSICAL AND SENSORY REQUIREMENTS: nn nnSit up to 2 hours per day; 1 hour at a time.

Stand for up to 10 hours per day; 8 hours at a time.

Walk 6 hours per day; 10 minutes at a time.

Consistently lift, carry and push objects up to 10 lbs.

Occasionally lift, carry and push objects up to 75 lbs.

Transports patients weighing up to 500 lbs.

via wheelchair, bed and/or stretcher.

Frequently stoop and bend.

Frequently reach above should level.

Must be able to perceive attributes of an object through touch.

Must be able to hear as it relates to normal conversation and high and low frequencies.

Must be able to see as it relates to general, near, far, color and peripheral vision.nn nn EDUCATION: nn nnCurrent RN license in the State of Pennsylvania.nn nn TRAINING AND EXPERIENCE: nn nnOne year medical/surgical experience preferred.
