ResponsibilitiesFairmount Behavioral Health Hospital provides comprehensive behavioral healthcare for children, adolescents, adults, and senior adults suffering from mental health and addictive illnesses. The Social Worker is responsible for psychosocial assessments, individual, group and family therapy, participation in treatment team planning, processing commitments, discharge planning/case management service, crisis intervention and liaison services with referral sources and other departments within the hospital.QualificationsJOB SPECIFICATIONS:To perform this job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description. The specifications listed below are representative of the knowledge, skill, and ability to meet the minimum requirements for this position. Staff must demonstrate flexibility for reassignment to units providing care to varied populations including children, adolescents, and adults in an inpatient and partial hospital setting.Education: Masters’ degree in Social Work required.Experience: Prior experience working with a psychiatric behavioral health population preferred. Behavioral Health inpatient experience preferred.Licensure/Certification: PA Social Work License preferred.Knowledge: Knowledge of case management, discharge planning, group leadership skills, age-specific growth and development, crisis and behavior management, family therapy theory and practice.Job Specific Responsibilities:Performs and appropriately documents a comprehensive psychosocial assessment on assigned patients within 72 hours of admission with consideration to age, mental, social and physical status to include discharge planning needs.Integrates information obtained from family, significant others and/or primary caregiver into assessment.Completes family/collateral support contact in conjunction with psychosocial assessment. Documents reason for inability to contact family/collateral support and efforts to do so on a daily basis.Actively participates in development of master treatment plan by presenting pertinent information and conclusions identified through the assessment and by incorporating therapeutic interventions.Develops realistic and measurable goals, along with desired outcomes.Participates in ongoing reassessment of the patient and incorporates changes in the treatment plan at least every 7 days.Documents in progress notes the patient’s progress towards treatment plan goals.Communicates progress and activities related to the discharge and/or continuing care to the patient, family, treatment team, referral source and other individuals involved in the patient’s care.Ensures adequate disposition arrangements and schedules aftercare appointments. Provides patients and families with appropriate resources and assists in accessing resources.Coordinates and manages mental health commitment proceedings.Facilitates group sessions as assigned.Demonstrates ability to recognize different types of abuse/neglect of patients and makes necessary Childine/Adult Protective Services reports if needed.Participates in clinical supervision with assigned supervisor and/or provide mentorship/supervision to junior colleagues or students.Completes all assigned trainings as required by the facility.Displays Service Excellence standards and promotes Mission and Values of the facility.Training/Competencies: Handle with Care, Verbal De-escalation, CPR, EMTALA, Hand Hygiene, Assessment and Prevention of Risk, Patient Observation Rounds, Therapeutic Boundaries, Therapeutic Milieu, Infection Control, Informed Consent, Circumstantial Life Changes, Patient Bathroom Competency, Code of Conduct, Group Expectations, Patient Advocate, Patient Belongings, Fire Safety, Workplace Violence, Patient Safety Discharge Process, All Staff Safety Test, All Staff Clinical Safety Test, Documentation and other assignments as assigned by the facility.Environmental Conditions: Exposure to psychiatric patients who may exhibit violent/aggressive behavior. Potential for exposure to communicable diseases, blood and body fluids, and other hazardous waste.Physical Requirements: Ability to communicate verbally and in writing. Physical agility to manage patients. Push, pull, and lift up to 40 pounds. Intermittent sitting, standing, walking, bending, and stooping.
