Case Manager – Medical Services

This is a Full-time position in Washington, PA posted April 29, 2021.

The Case Manager supports the Medical Services Team within a government agency, in planning and coordinating services to meet the health needs of individual program participants, and to promote quality and cost-effective outcomes in the participant’s best interest while in care and upon release.Duties include but not limited to:Work closely with the Medical Services Team on identifying and tracking individuals with complex medical and mental health conditions and individuals that require health care services/treatment while in care and/or post releaseCollaborate with care provider staff to include clinical, medical and case management staff involved in individual cases to assess and plan for treatment and discharge planningEstablish and maintain relationships with federal, state, local, and non-profit health agencies to coordinate the transition of medical services for individuals while in care and upon releaseCreate and sustain partnerships with community resources and support agencies, as well as coordinating with post release servicesMaintain a database of charitable and pro-bono services for program participants upon release from careMaintain a database of national health insurance programs that provide coverage for uninsured/ineligible program participantsSupport a quality assurance/process improvement program for the health programParticipate in medical site visits and needs assessments, as neededTrain and provide technical consultation to government staff, grantees, and other stakeholders on medical case management on a variety of topics, such as:Performing assessments of the physical, functional, social, psychological, environmental, learning and financial needs of patients that are elevated to headquarters for reviewIdentifying problems, goals and interventions designed to meet patient’s needsFormulating care plansProviding appropriate interventions which demonstrate knowledge of and sensitivity toward cultural diversityAssessing the patient’s formal and informal support systemsImplementing and monitoring the care plan to ensure the effectiveness and appropriateness of servicesEvaluating patient’s progress toward goal achievementResearching alternative treatment optionsActing as a patient advocate and assist with problem solvingProviding referrals to appropriate community resourcesQualifications5-8 years of relevant experienceStrong knowledge of pediatric health issuesStrong analytic capabilities and writing skillsStrong organization and communication skillsBackground in program case management, quality assurance or continuous program improvement, data analytics, research methodologyQualifications of flexibility, collegiality, creativity, and motivationCommitment to serving at-risk individuals and communitiesExperience or expertise in immigrant healthProven track record of program managementA minimum of bachelor’s level training in nursing (BSN) or clinical social workCase Management certificationMaster’s level Social work degree (LCSW) or Master’s degree in Public Health preferred.This position requires a Public Trust background investigation.
